A fuel filter is pretty cheap might wanna try changing it out. My truck ran very similiar to yours before i changed mines out.

you might wanna check the MAF door to make sure it moves freely, also check for vacuum leaks or any intake leaks after the MAF.
do you have any intake mods done to your truck?

sounds like your rocker arms need to be adjusted, loose timing chains make a terrible noise when you cld start the truck but goes away soon after....
