im assuming this is a 2wd correct?

is it constant? low speed high speed? variable? you sure the driveshaft is not damaged or dented? may be off balance

Vibrations are hard to diagnose over the internet. Is it shaking from the middle the truck ... or at one of the corners.... is it a specific speeds versus just high speed?

Where is it shaking at exactly? If it's the motor itself then it's just the motor mounts which is no biggie usually but the motor shouldn't be vibrating if you only touched the tranny. Try to record a video of it.

Could be the driveshaft angle(s). I dunno about the transmissions length difference... But you said you had the driveshaft extended and balanced. Hopefully that was all done right. Are you lowered?
