have u looked for a crack between 2nd and 3rd cylinder? anything new wrong with the truck? other than that check for leaks from the turbo to the...
the injector plugs only can only really go on one way, i say that becuase of the wire lengths. are u getting spark? have u double checked to make...

Stock, or modded? but it might be that the exhaust manifold has a crack in it. the manifolds are defective from the factory..

the one with the bolt is from the altenator. i just moved it, thats all makes for a good spot to and power for assories. the eye lid one is also...
the extra cabs back windows are taller than the reg cab trucks. im not positive on the older extra cab vs newer extra cab though..hope this helps...
